When you start preparing for MRCP Part 1, your first instinct is likely to grab a massive internal medicine textbook and start reading from page one. You probably start with Cardiology, move on to Respiratory, and hope to finish Neurology before the exam date.

As a clinical educator, I see this happen every diet. And every diet, I see brilliant junior doctors fail because they studied inefficiently. To pass MRCP Part 1, you must study strategically, and that means worshipping at the altar of the Official MRCP Blueprint.

What is the Blueprint?

The Royal College of Physicians publishes a blueprint that explicitly states exactly how many questions will be asked from each medical specialty. Out of the 200 total questions, the breakdown is not even. If you spend three weeks studying Dermatology (which has 8 questions) but only three days studying Clinical Sciences, you have set yourself up to fail.

The "Big Five" Clinical Specialties

The core of your clinical study time should be heavily weighted towards the "Big Five". Each of these specialties accounts for exactly 15 questions (75 questions total):

  • Cardiology: Heavy focus on ECG interpretation, heart failure management, and valvular diseases.
  • Respiratory Medicine: Know your BTS/SIGN guidelines for Asthma and COPD inside out.
  • Gastroenterology: Liver disease, IBD, and interpreting complex LFTs.
  • Neurology: Stroke localization, movement disorders, and cranial nerve palsies.
  • Rheumatology: Often surprising to candidates, Rheumatology is tested just as heavily as Cardiology. Master your autoantibodies and DMARD side effects.

The Hidden Kingmaker: Clinical Sciences

While the Big Five are crucial, there is one section of the blueprint that dwarfs them all. Clinical Sciences accounts for 25 out of the 200 questions (12.5% of the exam).

Ignoring or under-preparing for Clinical Sciences is the single most common reason competent candidates fail to reach the pass mark. It is sub-divided into:

  • Cell, Molecular, and Membrane Biology (2 questions)
  • Clinical Anatomy (3 questions)
  • Clinical Biochemistry and Metabolism (4 questions)
  • Clinical Physiology (4 questions)
  • Genetics (3 questions)
  • Immunology (4 questions)
  • Statistics, Epidemiology, and Evidence-Based Medicine (5 questions)

How to Attack Clinical Sciences

The questions in this section don't rely on clinical pattern recognition (like spotting a pulmonary embolism); they rely on hard, basic science mechanisms. Most of this is "forgotten knowledge" from your first two years of medical school.

Do not skip Statistics. The 5 statistics questions are often straightforward calculations (Sensitivity, Specificity, NNT, ARR) or test selections. If you memorize the formulas, these are guaranteed marks.

Focus on Immunology and Genetics. The College frequently tests inheritance patterns (Autosomal Dominant vs Recessive vs X-Linked) and hypersensitivity reactions (Types I-IV). Match the clinical presentation with the underlying immune defect (e.g., Anti-dsDNA in SLE).
